Ayushman Card: Check Eligibility and Download Online (PM-JAY)
The Ayushman card (formerly the “golden card”) under Ayushman Bharat PM-JAY gives eligible families free treatment of up to ₹5 lakh per family per year at empanelled government and private hospitals. Since 2024, all senior citizens aged 70 and above are also covered, regardless of income, under the Ayushman Vay Vandana card. Here’s how to check eligibility and download your card for free.
Who is eligible
- Families identified in the SECC 2011 database and those added by state governments
- Many state health scheme beneficiaries (integrated with PM-JAY in most states)
- Every citizen aged 70+ (Ayushman Vay Vandana), with a separate top-up cover for those already in PM-JAY families
How to check eligibility
- Go to beneficiary.nha.gov.in (or open the official Ayushman app).
- Log in as Beneficiary with your mobile number and OTP.
- Select scheme PM-JAY, your state and district.
- Search by Aadhaar number, family ID or name.
- If your name appears, you’re eligible.
How to download your Ayushman card
- After searching on beneficiary.nha.gov.in, find your name in the results.
- If eKYC is complete, click Download Card. If not, complete Aadhaar eKYC first (OTP or face authentication), then download once approved.
- Save the PDF. You can also show the card in the Ayushman app.
Making and downloading the card is free. You can also get help at a Common Service Centre or the Ayushman Mitra desk at an empanelled hospital.
Using the card in hospital
- Go to an empanelled hospital and visit the Ayushman Mitra desk.
- Carry your Ayushman card and Aadhaar.
- Treatment for covered packages is cashless.
